Application Forms

Important Note: Following are most of the the application forms the JASIN Program uses. Each applicant does not have to submit all of these forms. In order to know which forms are necessary for your application, follow these intructions according to your type of application:

1. Applicants through the University of Wisconsin-Platteville's Institute for Study Abroad Programs.

Receive application materials from UWP's office and follow their instructions regarding application materials.

2. Applicants from Affiliated Institutions 

Receive application materials from your home university's study abroad office and follow their instructions regarding application materials.

3. Part-time study applicants

The application materials for part-time study are not available here. Please contact the International Center to request part-time study application forms.

 For those wishing to apply for the scholarship from the Japan Student Services Organization (JASSO) please have your study abroad or academic advisor fill in the following form and include it with your application materials. No scholarships are available until Fall Term 2010.
